What strategy has proven to be most successful in increasing vaccine rates in your organizations and communities?
Concerning the COVID vaccine, I think it's the general hope that we can move forward once people are vaccinated- for the residents, it's for their safety and so they can see their families again.

Other- I am a LNHA but currently not working in a facility. I appreciate keeping up with SNF QAPI.

So many - decreasing readmits is currently one of our ACO's toughest initiatives
Does your local community have a community-wide coalition for working together to impact hospital readmissions and care transitions?
We have implemented the use of SBAR in our community and in the process of reinvigorating the use of the valuable tool. Change management and sustainability is always something to tackle. :-)
Unsure about any other community coalitions, but would be interested in how to set one up
