As the clinic pharmacist at a small rural hospital / outpatient clinic, due to staff changes, I am going to start taking a more proactive role in our transitional care management process. I attend daily rounds with the hospital staff and we work together to identify patients who could benefit from additional pharmaceutical management, and they refer those patients to see me
